Vision Correction - Online Self-Evaluation

This brief self-evaluation will help determine your candidacy for vision correction care, and may help guide the appropriate selection among several good treatment options. The questions should take no more than a few minutes to complete. When you are done, simply hit the "Get Your Results" button and we will provide a preliminary (automated) response.

If you have specific questions you would like answered, you will have the opportunity to directly contact our vision correction team after submitting your self evaluation.

  4. Does your Drivers License carry the restriction "Must wear corrective lenses?"

  5. Are you in good general health?

  6. Have you ever had any eye injury, corneal surgery, serious corneal infection, or been told you have keratoconus?

  7. Do you currently wear reading glasses?

  8. How interested are you in seeing well up close (reading) without glasses?

  9. If you were determined to be a good candidate, how soon would you like to have treatment?
